Home from Hospital has been helping people since 1994. It was set up in order to help people to manage in their own homes following discharge from hospital.

You may have had elective surgery - for example a hip replacement – or you may have had a hospital admission following a fall or accident. Whatever the reason for your hospital stay, you may feel that you require a little extra help when you return home.  We are here to help you.

We provide a free service for up to six weeks following hospital discharge. Our volunteers are well-trained and supported in the tasks that they undertake.

Examples of what we can do are:

  • changing anti-embolism compression stockings
  • shopping
  • befriending
  • referring to other agencies (Eg to the DWP for appropriate benefits,
  • or to Social Services for assessments including Carers’ Assessments)

Our aim is to help you get back on your feet and to feel confident enough to resume your usual activities.

Referrals to Home from Hospital can come from any source, including self-referrals, but generally come from hospital staff – eg ward nurses or OTs.
