Home from Hospital

Helping people to remain in their own homes.

Our Origin

Home From Hospital Service originated, from a research audit and needs analysis undertaken in 1993/94. The community audit was completed by 5000 households and informed the structure and content of the project – i.e. free short-term 6-8 week support for older people on discharge from hospital: The aim to assist vulnerable people through a period of convalescence to continue confidently in their own homes.

We are a volunteer led organisation and our volunteers who are members of the local community support their neighbours to regain and maintain their independence, allowing them to be in control of their lives.

Since those beginnings, the service has grown and now, as well as the Adult Service, we also offer a Carer's Support SchemeFamilies Together and a TeleHealth Service.

All services and projects are maintained through constant recruitment and training of volunteers.  All volunteers are DBS checked, trained in health and social care skills, and are fully insured. Many move on to full employment or higher education and they receive an excellent quality of education and support from qualified staff.
